Insurance

For patients with private insurance or a government sponsored plan (Medicaid), using your insurance benefits is often the best way to pay for laboratory testing. To ensure that insurance billing is the right choice for you, our dedicated billing team is here to help determine coverage and estimate costs upfront.

  • Benefit Investigation

SDxLabs will perform a benefits investigation to determine if your test is covered by your insurance provider. If your insurance does not cover your test, our billing team can help you determine if a self-pay option is right for you.

  • Assistance with prior authorization

If prior authorization is required before testing, our billing team will work with your provider to obtain necessary supporting documents and submit it to your insurance company.

  • Pretest cost estimate

Even if your insurance covers your test, some patients may still have an associated out-of-pocket cost depending on their deductible or other plan specific requirements. We will provide you with an estimated cost and, if this number is higher than expected, we can review other billing options.

Self-Pay

The self-pay billing option is typically used by people who have high out-of-pocket costs associated with insurance billing, do not have test coverage by their insurance provider, or do not have insurance.

If the self-pay option is selected, SDxLabs will not bill your insurance for testing. As such, the amount you pay will not go towards your insurance deductible or out-of-pocket maximum. Payment information (credit card, HSA card or FSA card) must be provided prior to the start of testing. To submit your payment information, please call (412) 677-0664.

Financial Assistance

We understand that paying for laboratory tests may seem out of reach for some patients. To help eliminate financial stress associated with testing, we are proud to offer our financial assistance program. This program is designed to help eligible patients who are uninsured, underinsured, or who face financial hardship, by offering reduced out-of-pocket costs.

Our financial assistance program reduces out-of-pocket costs for qualifying patients utilizing our self-pay billing option. Cost reductions are on a sliding scale based on family/household size and annual gross income*.  To apply for our financial assistance program or to learn more, please call (412) 677-0664.

* Qualifying incomes per household/family are based on the United States Department of Health & Human Services (HHS) Poverty Guidelines. Annual gross income includes the following for all members of your household: Gross Salary, Unemployment Compensation, Disability and Worker’s Compensation, Social Security and/or Supplemental (SSI) Benefits, Public Assistance (TANF, SNAP, etc.). Income values provided are for residents in the 48 contiguous states and D.C. For qualifying income criteria for residents of Alaska or Hawaii or for families with greater than 7 members, please call (412) 677-0664.
